Levetiracetam is a broad-spectrum antiepileptic drug (AED) with a unique mechanism of action, often used as monotherapy or adjunctive therapy for various seizure types.

Mechanism of Action

The precise mechanism of action is not fully understood, but it is thought to involve binding to the synaptic vesicle protein 2A (SV2A) in the brain. This binding modulates neurotransmitter release, particularly glutamate, and helps to normalise neuronal excitability without directly affecting ion channels or GABAergic transmission.

Key Indications

Levetiracetam is indicated for the treatment of focal (partial) seizures, myoclonic seizures in juvenile myoclonic epilepsy, and primary generalised tonic-clonic seizures. It is often considered a first-line agent due to its broad spectrum of activity, favourable pharmacokinetic profile, and relatively low potential for drug interactions.

Contraindications

Absolute contraindications are few, primarily hypersensitivity to levetiracetam or other pyrrolidone derivatives. Caution is advised in patients with severe renal impairment, as dose adjustments are necessary. It is generally well-tolerated and has a relatively low risk of serious adverse effects compared to other AEDs.

Adverse Effects

Common adverse effects include somnolence, dizziness, asthenia, and irritability. Behavioural and psychiatric adverse effects, such as aggression, agitation, depression, and mood changes, are notable and can be dose-dependent. Serious but rare effects include Stevens-Johnson syndrome and toxic epidermal necrolysis, though less common than with other AEDs.

Monitoring

Monitoring is primarily clinical, assessing seizure control and the presence of adverse effects, particularly behavioural changes. Renal function should be monitored at baseline and periodically, especially in patients with pre-existing renal impairment, as dose adjustments may be required. Therapeutic drug monitoring is generally not routinely necessary.

Counsel patients about common side effects like drowsiness and dizziness, and importantly, the potential for mood changes, irritability, or aggression. Advise them to report any such changes to their doctor. Emphasise that it can be taken with or without food and has fewer drug interactions than many other AEDs.
