Drug Class & Overview
Naloxone is a pure opioid antagonist. It rapidly reverses the effects of opioid agonists by competing for and displacing them from opioid receptors.
Mechanism of Action
Naloxone acts as a competitive antagonist at all opioid receptors, particularly the mu-opioid receptor. It has a higher affinity for these receptors than most opioid agonists, thereby reversing their effects, including respiratory depression, sedation, and analgesia.
Key Indications
Naloxone is indicated for the complete or partial reversal of opioid-induced respiratory depression and central nervous system depression. It is used in the emergency management of suspected opioid overdose. It can also be used to diagnose opioid dependence in a comatose patient.
Contraindications
The only absolute contraindication is hypersensitivity to naloxone. However, caution is advised in patients with pre-existing cardiovascular disease or those who have received large doses of opioids, as rapid reversal can precipitate acute opioid withdrawal syndrome, potentially leading to cardiovascular complications.
Adverse Effects
Rapid reversal of opioid effects can lead to an acute opioid withdrawal syndrome, characterised by nausea, vomiting, sweating, tachycardia, hypertension, and agitation. Pulmonary oedema, cardiac arrhythmias, and seizures are rare but serious adverse effects, particularly with rapid administration in opioid-dependent individuals. Pain may return abruptly.
Monitoring
Following naloxone administration, continuous monitoring of respiratory rate, oxygen saturation, heart rate, blood pressure, and level of consciousness is essential. The patient should be observed for recurrence of opioid depression, as naloxone's duration of action is often shorter than that of the opioid. Repeat doses may be necessary.
Prescribing Safety (OSCE)
In an emergency, naloxone should be administered without delay. Ensure appropriate monitoring equipment is available. Counsel family/carers on the signs of opioid overdose and how to administer naloxone if a take-home supply is provided. Emphasise the need for emergency services even after administration.
