Drug Class & Overview

Clopidogrel is an antiplatelet agent belonging to the thienopyridine class. It is a prodrug that inhibits platelet aggregation.

Mechanism of Action

Clopidogrel is a prodrug that requires hepatic metabolism by cytochrome P450 enzymes (primarily CYP2C19) to its active metabolite. This active metabolite irreversibly blocks the P2Y12 adenosine diphosphate (ADP) receptor on the surface of platelets. Blocking this receptor prevents ADP-mediated activation of the glycoprotein IIb/IIIa complex, thereby inhibiting platelet aggregation.

Key Indications

Clopidogrel is used for the prevention of atherothrombotic events in patients with a history of myocardial infarction, ischaemic stroke, or established peripheral arterial disease. It is also indicated, often in combination with aspirin (dual antiplatelet therapy - DAPT), for acute coronary syndromes (unstable angina, NSTEMI, STEMI) and following percutaneous coronary intervention (PCI) with stent placement.

Contraindications

Absolute contraindications include active pathological bleeding, such as peptic ulcer or intracranial haemorrhage. Severe hepatic impairment is also a contraindication. Caution is advised in patients at increased risk of bleeding, including those with a history of GI bleeding or trauma, and those undergoing surgery.

Adverse Effects

Common adverse effects include bleeding, ranging from minor bruising to serious haemorrhage. Gastrointestinal disturbances such as abdominal pain, dyspepsia, and diarrhoea can occur. Skin rashes are also reported. Rarely, serious side effects include thrombotic thrombocytopenic purpura (TTP) and agranulocytosis.

Monitoring

No routine blood monitoring is typically required for clopidogrel. Patients should be monitored for signs of bleeding, particularly during the initial weeks of treatment or when co-administered with other antiplatelet or anticoagulant agents. Liver function tests may be considered in patients with pre-existing hepatic impairment.

Prescribing Safety (OSCE)

When prescribing, always check for active bleeding and a history of bleeding disorders. Be aware of drug interactions, particularly with proton pump inhibitors (PPIs) like omeprazole (due to CYP2C19 inhibition, potentially reducing clopidogrel's efficacy), and other antiplatelet agents or anticoagulants. Counsel patients on the increased risk of bleeding and to report any unusual bruising or bleeding.
